Referral and Criteria

To be eligible for the Yolo County Nurse Home Visiting Program, participants must:

• Live in Yolo County
• Be pregnant or parenting a child under the age 2 1/2
• Be low-income

To be eligible for the Yolo County Adolescent Parenting Program, participants must:

• Live in Yolo County
• Be a pregnant or parenting adolescent under the age of 19 with a child under the age of 2 1/2

In addition, the participant may meet the following criteria:

• Birthing parent or infant with a high risk or complicating medical condition such as gestational diabetes or recently discharged from the NICU
• History of previous poor birth outcome, for example: prematurity, fetal or neonatal demise
• Family history of child neglect or abuse (Birthing parent or baby)
• Birthing parent mental health and/or alcohol and drug problems

Families that do not meet the above criteria but who's primary care provider feels would benefit from public health nurse case management may be accepted as space allows.  For example:

• Parent with mental health and/or alcohol and drug problems with children until the age of 5
• Children under age 5 with poor physical health, social-emotional and/or developmental problems
