This document covers Phase I of the CHIP and outlines plans for Phase II (CHIP Implementation). The purpose of identifying two phases is to acknowledge and document the extensive efforts of CHIP development workgroup in identifying priority areas and developing a menu of potential objective themes, strategies, and metrics as Phase I, and then to recognize that further refinement of the objectives, strategies, and measures will be needed in Phase II to implement and evaluate effective interventions.
Phase II will begin with a relaunch Healthy Yolo in early 2024. Healthy Yolo will continue to be a collaborative that includes community agencies, government, and community members. HHSA will backbone Healthy Yolo, and the structure will include a leadership body as well as partners working on implementation of specific interventions.

Community Health Assessment (2023-2025)

The Yolo County Health and Human Service Agency’s (HHSA) Public Health Branch has released the 2023-25 Community Health Assessment (CHA). The CHA includes information about the overall health of residents in the county and identifies eleven significant health needs. A CHA is released every three years. It is used to help the County, alongside community, and partners, make decisions about how to improve health outcomes for residents. The CHA also contains information about the social determinants of health, such as education and income levels, and how they impact the health of various groups in the county. The CHA is built upon a three-year Yolo County Community Health Needs Assessment (CHNA), written in collaboration with CommuniCare Health Centers, Dignity Health, Sutter Health, and Winters Healthcare, with support from Community Health Insights.
Community Advisory Program
Residents ages 14 years and older were invited to join the CHIP Workgroup as Community Advisors through the pilot Community Advisory Program (CAP). Community Advisors received a stipend for their participation. For additional information, see: Community Advisory Program.
